Suitability Questionnaire

This suitability review will ensure that the recommendations, products and service that CPW give to you are still appropriate, and it is vital that you are an integral part of this review process. The following questions in the following areas will help assess suitability.

• The term of your product(s) is/are still appropriate. This currently relates to when the product was originally agreed.

• Your attitude to risk is still what was originally agreed.

• You have an adequate emergency fund.

• An assessment of your current assets to see if you have gained more wealth.

• Any substantial changes to your liabilities that would require a review.

• Any change in your income.

• Any change in your expenditure or outgoings.

• Any change in your life that would class you as potentially vulnerable.

We will ask you some simple questions about your current situation. This is to make sure that when we originally set up your investment, it is still the right product for your circumstances. You do not need any additional information to complete the assessment and it will only take a few minutes.

Complete our Questions

1. Do you feel the term of your Investment plan (i.e. ISA, OEIC, Investment Bond) is still appropriate? Required
2. Do you feel the term of your Pension plan (i.e., Personal Pension, Drawdown, SIPP) is still appropriate? Required
4. Are you still comfortable with the level of risk applied to your Investment/Pension plan? Required
5. Do you have an adequate emergency fund that is easily accessible? (For example, in a bank account you can access quickly) Required
6. Are there any changes to assets and other investments? (For example, have you acquired another property, gained cash savings or other investments, or received an inheritance or windfall?) Required
7. Has there been any substantial changes to your income or expenditure that would affect your affordability (For example, has your debt increased or has there been a change in family or personal circumstances?) Required
8. Do you expect or have there been significant changes in your life such as family bereavement or illness, or substantial changes in personal circumstances that would affect your decision making when we review your investments? Required
9. Is there an amount of capital that you wish to consider investing at present? Required
10. Are you saving regularly at present or is there an amount that you wish to invest on a monthly basis, at present? Required
